What is Upper Crossed Syndrome? By Dr. Trevor Wendel DC Upper Crossed Syndrome (UCS) is a phrase you may have heard recently from personal trainers or physical therapists. What it refers to is not a disease, but rather a pattern of muscular imbalances in the neck and shoulders.... UCS is a postural issue generally seen among people who work at a computer desk for a living, and typically involves the shoulders migrating up towards the head and rounding forward, and the neck and head migrating forward towards the screen or whatever we are looking at. Does this sound familiar? If we pick apart this muscular pattern, we will find which muscular are contracted (tighter) and which ones are elongated (loose). The pectoral muscles, the largest muscles that make up the front of our shoulders are typically tight and contracted, and because they are a lot stronger than the muscles of the neck and the mid-back, they draw the shoulders forward, and weaken the muscles between the shoulder blades (the rhomboids, and serratus anterior). In addition, because we have to hold our head up to see our computer screen, the upper trapezius and levator scapulae muscles which attach to the sides of the neck become tight and rigid as well. What can we do to fix this? When we recognize that UCS is simply due to a muscular pattern or posture that we have adopted over time, we can take steps to retrain the body. The first step to take is to stretch out the muscles which are tight and contracted. Our pectoral muscles can be easily stretched out using a doorway, by bending both arms 90 degrees at the elbow, placing the hands on the doorway at a position a little above the head, and gently leaning into the doorway. The pectoral muscles may feel surprisingly tight when we do this stretch. Hold the stretch and breathe for 15 seconds, and repeat 3 times. The next step is to engage and strengthen the muscles of the neck and mid-back which have become weak. A simple exercise that achieves this is called a ‘Bruegger’. It is best done in a standing position, 1) tuck your chin into your neck as if you were trying to press the back of your head against a wall. 2) drop your shoulders down. 3) point your chest up and out. 4) bring both arms behind you, and attempt to point your thumbs directly behind you. You should feel your shoulder blades squeezing together, and the muscles in the area engaging. Hold for only 5 seconds, and repeat 3 times. Chiropractic care and massage therapy are also an important and effective way to retrain the body into adopting a new posture.
